Summary


We are looking for a dynamic and patient focused Advanced Clinical Practitioner with a specialist Interest in neurological rehabilitation. You will join our multidisciplinary team in Danesbury neurological centre in Welwyn. Danesbury is a 24 bedded Neurological Centre and takes patients on a 35-42 day pathway. You will work alongside the ward Leads to provide leadership and expert clinical knowledge for the inpatient unit and identifying and taking forward quality improvement. You will deliver high quality, highly specialist clinical care to patients within scope of professional practice. You will be required to work alongside the medical team in reviewing and presenting clear medical plans in the MDT. We will consider applicants who wish to work full or part time and we can be flexible with the working pattern.
• Be the clinical lead for neurological patients.
• Carry out weekly ward round alongside the ward GP and Speciality doctor.
• Prescribe and review medication as per required/appropriate for patients on the unit.
• Interpret/request blood tests where appropriate for patients on the unit.
• Interpret/request other investigations.
• Develop medical treatment plans for patients on the unit.
• Provide supervision and support for the nursing team on the unit.
• Coach staff both internal to HCT and external seeking advice on patients’ suitability for admission to the unit and liaise with the acute trusts.
• Support in the development/training of ACPs across HCT.
• Work alongside the project development team in HCT.
• Communicate with other HCT Inpatient units to ensure appropriate medical/ACP cover across all three sites.

The post holder will apply advanced clinical expertise, levels of judgement, discretion and decision making. Will act as an autonomous practitioner working independently and will exercise expert clinical practice in assessment, diagnosis, prescribing and development of medical treatment plans for patients with neurological needs, complex presentations and multi pathologies.

The role will provide an expert resource, providing a visible clinical profile, having both advisory and clinical input into patient care. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.

Works collaboratively with the multi-disciplinary team including medical, nursing and allied health staff to manage medical conditions and to plan and implement treatment for illnesses.

As an Advanced Clinical Practitioner, this role will provide education to other staff and students within the clinical area.
