Summary
To compliment your role as a Health Visitor, you will: Focus on health at a population level, looking at ways to make communities and environments healthier, reducing ill-health and tackling health inequalities Measure, monitor and report population health and wellbeing; health needs, risks, and inequalities; for a specific chosen area of your work Work with the Trust Public Health team and existing childrens services to promote population and community health and wellbeing, addressing the wider determinants of health and health inequalities Audit, evaluate and re-design services and interventions for a specific chosen area of your work to improve health outcomes and reduce health inequalities Work with, and through, policies and strategies to improve health outcomes and reduce health inequalities Work collaboratively across agencies and boundaries to improve health outcomes and reduce health inequalities for a specific chosen area of your work Provide leadership to drive improvement in health outcomes and the reduction of health inequalities Please see attached Job Description and Person Specification for full roles and responsibilities
