Summary
As a dual diagnosis lead for the CMHT , the post holder will ensure that people with substance misuse problems are supported to engage with community drug and alcohol teams. To support service users to improve general health, including dietary needs and personal hygiene. To review and assess the needs of service users with a dual diagnosis of Serious Mental Illness(SMI) and Substance use. To provide support in the community for individuals and families with the aim of helping them to make the positive lifestyle changes that will aid their recovery.To support and signpost individuals and families to engage with other supportive services. To be proactive in reducing harm in line with the recovery agenda and advocate for service users in addressing their physical, mental health and social care needs To support individuals and families to access appropriate treatment and recovery services.
