**Job Title:** Personal Advisor (Children's Services - Leaving and Aftercare)

**Role Clarification

This role is part of a 16 Plus Team dedicated to supporting Children Looked After (CLA) and Care Leavers during their transition to adulthood. The Personal Advisor will play a crucial role in providing comprehensive support to care leavers as they navigate the challenges of independence, ensuring they have the necessary resources and guidance to thrive. This includes working closely with Unaccompanied Asylum-Seeking Children (UASC) and other vulnerable groups. The Personal Advisor will be responsible for assisting care leavers in all aspects of their lives, from housing and education to employment and emotional well-being.

**Key Responsibilities:**

- Provide individualized support and advocacy for care leavers as they transition from care to independence, addressing their holistic needs and aspirations.
- Collaborate with the 16 Plus Team and other relevant stakeholders to develop and implement effective support plans tailored to the unique circumstances of each care leaver.
- Support care leavers in accessing housing, education, employment, healthcare, and other essential services, ensuring they have the necessary resources to succeed.
- Assist care leavers in developing essential life skills, such as budgeting, cooking, and self-advocacy, to enhance their independence and self-sufficiency.
- Conduct regular assessments to identify the evolving needs and goals of care leavers, adjusting support plans as necessary to ensure their continued progress and well-being.
- Provide emotional support and crisis intervention to care leavers facing challenges or difficulties, empowering them to overcome obstacles and build resilience.
- Advocate for the rights and interests of care leavers within the care system and broader community, ensuring their voices are heard and their rights are respected.
- Maintain accurate and up-to-date records of all interactions and interventions with care leavers, adhering to departmental policies and procedures.
- Stay informed about relevant legislation, policies, and best practices related to leaving care and support services, integrating this knowledge into practice to enhance service delivery.
- Collaborate with colleagues and external partners to facilitate smooth transitions for care leavers, promoting seamless continuity of support as they move towards independence.
The Personal Advisor role within the 16 Plus Team is integral to supporting care leavers during a critical period of transition in their lives. Candidates should possess a strong understanding of the needs and challenges faced by care leavers, as well as experience in working with teenagers in a statutory setting. Knowledge and experience of working with Unaccompanied Asylum-Seeking Children (UASC) is essential. The ability to complete Pathway Plans and advocate for the rights of care leavers is paramount. A car is essential for this role, and candidates should be prepared for office-based work on designated days.
