Summary

The post holder will support the Head of Department and their team to ensure that the All Age Continuing Care Teams work-streams are managed effectively in relation to implementation of the National Framework for NHS Continuing Healthcare (CHC) & NHS Funded Nursing Care (FNC). Assessment and reviews of NHS CHC patient pathways where the responsible commissioner is NHS Kent and Medway Integrated Commissioning Board (ICB). Care settings include specialist residential, supported living and nursing homes. The post holder is required to work in partnership with NHS colleagues, Local Authority and private sector organisations as appropriate with the aim of ensuring that treatment timelines are adhered to and clinical goals are met. The post holder will undertake comprehensive, holistic assessments and reviews of individuals using appropriate assessment tools to implement the National Framework for Continuing Healthcare and Funded Nursing Care. Manage enquiries by various stakeholders in relation to, NHS CHC, FNC and provide detailed, informed verbal and written responses. Assist the senior management team in order to ensure correct processes are followed in delivery of the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care. Represent the department at Local Authority (LA) safeguarding meetings. Ability to undertake assessments of capacity and coordinate/complete Best Interest Meetings as required. Ensure accurate and open communication with a range of organisations and individuals. Keep accurate and robust records at all times.
