Summary

Carers Liaison Worker Vacancy

18 hours per week £30055 pro rata + 5% employer pension contribution

We are looking for an experienced and highly motivated individual with an understanding of the needs of unpaid carers to join our team. As well as providing emotional support, advice, advocacy and information, the successful candidate will be responsible for carrying out Carers Support Plans primarily in the Tweeddale locality of Scottish Borders.

As this post will be primarily home-based, candidates will also need to demonstrate initiative and the ability to organise their workload.

Driving license and access to a vehicle desirable – mileage rate paid.

Knowledge of Community Care and Carer Legislation is essential.

JOB PURPOSE

To provide advice, information, advocacy and support to adult carers in the Tweeddale area

MAIN DUTIES & RESPONSIBILITIES

To provide one-to-one emotional, practical and advocacy support to carers
To provide information and advice
To assess carers’ needs and complete a Carers Support Plan
To signpost and refer carers to relevant statutory and voluntary services
To work within a quality assurance framework and keep accurate and confidential carers records
To improve the lives of unpaid carers
To provide access to funding and respite opportunities
This job description is indicative of the nature and level of responsibilities associated with this job. It is not exhaustive and the job holder may be required to undertake other duties and responsibilities commensurate with the grade and in discussion with the line manager.

Management & Accountability

Be accountable in the first instance to the Team Leader
Prepare and agree with the Team Leader a programme prioritising areas of work
Participate in staff planning meetings
Participate in and contribute to regular training, support and supervision and annual appraisal meetings.
Skills and abilities

ESSENTIAL

Educated to Higher grade level, equivalent or above

An understanding of and commitment to the needs of carers

Good written and oral communication and presentation skills

Excellent interpersonal skills

Ability to demonstrate initiative, develop and implement new ideas and organise/prioritise own workload

Be able to work as part of a team and support equal opportunities

Be able to work flexibly to meet the needs of carers and the service

Knowledge and understanding of the principles, values, policy and practice of community care particularly in relation to unpaid carers

High level of IT and database skills
