Are you ready for a career change that's fulfilling and rewarding? This is your chance to join our Health and Social Care Partnership as a Social Care Officer, in the HART Plus team, who cover all over the Perth and Kinross area. Experience is not necessary as all training will be provided.

Our team provide early intervention and Crisis care to vulnerable service users across Perth and Kinross to allow them to remain living in their own homes for longer. This team specialises in end of life care. If you are passionate about making a positive difference to the lives of the most vulnerable people in your community, read on and find out more. The working pattern for this role is 12 hour shifts with a mixture of 08.00-20.00 and 20.00-08.00

Responsibilities

In this role:
• Visit service users by following a pre-planned schedule, to support with daily tasks, including personal care
• Ensure the health and safety of service users, yourself and colleagues in line with our policies and procedures
• Protect service users from risk and report any concerns to your line manager
• Maintain accurate records and provide updates on service users’ progress
• Comply with national care standards (National Care Standards- A Guide - gov.scot (www.gov.scot)) and the Scottish Social Services Council Codes of Practice
• Require to work flexibly, including evenings and weekends
• Have effective communication skills and use your initiative while working on your own
• Be enthusiastic, flexible and willing to learn
• A current driving licence and access to personal transport will be required due to the location of the role within the team.

The post holder will be responsible for: -

following a planned schedule of service user visits;
support service users with personal care tasks to enable the person to reach their optimum level of independence;
protect individuals from risk or abuse and to report any concerns to the HART Plus coordinators;
ensure Health & Safety for yourself and service users' in line with PKC policy and procedures;
participate in risk assessments;
maintain accurate records and report on service users' progress;
comply with the National Care Standards and SSSC Codes of Practice.

You will be required to register with the Scottish Social Services Council (SSSC)
