Summary


The role is based out of County Hospital or YYF and the post holder will travel across ABUHB in the community to assess and treat patients in their own homes. The post holder will work alongside a HCSW and is overseen by a Team Manager.

Assess and deliver care to patients in the community under the direction of the team leader.

Maintain accurate record keeping and standards for documentation including IT systems

Ability to work 12 hour shifts at night

Have competence in a range of clinical skills needed for community nursing.

The OOH Community team are based from County Hospital and YYF.

The night shift team cover all of ABUHB incorporating all the boroughs within the health board.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

To assess, plan, implement and evaluate care under the direction of the team leader.

To liaise with other members of the MDT in planning and delivering care.

To ensure the provision of effective, efficient, and high quality patient care during the hours of 19:30 - 08:00 seven days a week.

To have and develop clinical skills required for the role. The post holder will be supported with competencies and development.
