Summary

The Senior Exercise Referral Specialist (PSI) will be responsible for working closely with patients referred with complex long term health conditions, to improve their physical and mental health through accessing a condition specific tailored physical activity programme. In accordance with current NICE guidance the post holder will motivate and inspire individuals to achieve a long-lasting change and adherence to exercise using motivational interviewing. Also, support the team to be motivated and lead by example with good practice.

To Deliver FaME (Falls Management Exercise) a 6-month strength and conditioning program which provides clients/patients with a minimum dose of proven 50 hours of exercise, which has been shown to reduce falls by 35% to 54% respectively.
Multiple sessions will be provided across Droylsden and Denton using community venues that are easily accessible, goal is to deliver 8 sessions per week. Sessions will be able to support 12 clients per class which means this would be able to provide delivery of 4 cohorts a time servicing 48 clients per week which give a minimum of 198 clients within the area the opportunity to receive a proven pathway to improving strength and balance.
