Summary

A dynamic care provider in Crewe is seeking a part-time Community Care Support Worker to play a crucial role in supporting visits and care planning for an expanding portfolio of clients, whilst also contributing to the growth and development of the business.

The role offers flexible hours as well as excellent training opportunities and pay at £ 11.44 per hour and mileage allowance.

As the Community Care Support Worker, you will be responsible for:

* Providing practical and domestic services with designated clients as agreed in the Individuals Care Plan.
* Ensuring outcome-based support is provided, clients progress is monitored, tracked and reviewed.
* Liaising with relatives and informal carers, encouraging involvement in the client’s individual care and support plan.
* Building and sustaining professional relationships with key stakeholders including, local authority care managers, clinicians, GPs, Pharmacists and warden control centres where appropriate.
* Ensuring compliance with CQC standards, safeguarding and other regulatory bodies.
* Participating in personal development and training opportunities where possible to continue to develop knowledge and skills.

The Community Care Support Worker will have the following skills and attributes:

* Effective communication and influencing skills with the ability to build rapport quickly.
* Demonstrable background in community caregiving, with a focus on providing support in residential settings, especially with children and young people.
* NVQ Level 2 in Health & Social Care is desirable.
* Good knowledge of Essential Care Standards and other regulatory framework, and Health and Safety legislation is desirable.
* Full UK driving license and use of a vehicle is essential.

You will be working flexibly up to 16 hours per week from the office in Crewe and from different locations within the local community. Hours will be agreed during the interview.
