Summary


We have an exciting opportunity to join our team of Uro-Oncology Clinical Nurse Specialists (CNS) as a Cancer Support Worker. You will have excellent communication skills and work well as a core member of the team.

The band 4 Cancer Support Worker (CSW) role iscentral to the delivery of excellent patient care. You willwork closely with the CNS's, but also with other members of the multi-disciplinary team, such as nurses, AHPs, MDT co-ordinators and medics. You willassist the CNS's to co-ordinate the assessment of patient’s unmet needs, triage and co-ordination of referrals to specialist staff or into the community. You will provide a single point of access for information and support throughout the patient’s cancer pathway, working with patientsdirectly both face to face and over the phone assisting with information and support and co-ordination of a sometimes complex journey.

The role requires use of judgment in responding to the needs of individuals. The level of judgment required relates to identifying the complexity of the situation, providing appropriate advice, and escalating to the registered practitioner where appropriate within clinical algorithms. The CSW will be responsible and accountable for their practice and behaviour under the guidance and supervision of a registered practitioner.

You will work under the supervision of Clinical Nurse Specialists with the aim to release time for them to conduct the core elements of their role, it is envisaged that this will primarily be around triaging telephone calls, co-ordination of care, supporting Holistic Needs Assessment, and a Personalised Stratified Follow up Programme. Experience of caring for Urology patients would be an advantage.
