Summary


NHS@Home is an innovative service, providing care for people who are acutely unwell in their own homes across Bristol, North Somerset, and South Gloucestershire. It offers an alternative to hospital and helps people return home from hospital more quickly, too.

NHS@Home has five pathways, covering a wide range of conditions including frailty, respiratory, heart failure, and general medical & surgical recovery. Our team provide a very wide range of acute healthcare treatments within the home, in addition to monitoring health conditions and vital signs for deterioration.

Working our team you will utilise a wide set of acute care skills in the community setting. You will contribute to a multi-disciplinary approach, undertaking virtual board rounds and working alongside specialist doctors, advanced clinical practitioners, nurses, AHP's and pharmacists to deliver an effective treatment plan that will make a real difference to the lives of local people.
• Delivering care to patients in their own home across Bristol, North Somerset and South Gloucestershire based from either from the Weston General Hospital ORBristol Royal Infirmary (which ever is most convenient for you).

Working within the NHS@Home service, your duties will include:
• Providing acute care to patients with complex and specialist needs in the community, across Bristol, South Gloucestershire and North Somerset.
• Working alongside the multidisciplinary team to develop, implement and evaluate care plans.
• Assessing the health and psychosocial needs of the patient within the hospital and community setting.
• Plan and implement treatment and use a wide range of service to meet their needs.
• Utilising a wide range of clinical skills to support the ongoing management of patients in their home environment e.g. delivering IV therapies, venepuncture, cannulation and wound care (not exhaustive list).
• Communicating effectively, networking and liaising to ensure high quality responsive, safe, efficient and compassionate care is provided for our patients, carers and famili

The service uses a mixture of face-to-face visits, phone and video calls, and remote monitoring through digital technology, to help people recover safely in the comfort of their own home.

The service is run in partnership between Sirona care & health, North Bristol Trust, University Hospitals Bristol and Weston NHS Foundation Trust and BrisDoc.

The post holder will work in collaboration with the NHS@Home team at the Bristol Royal Infirmary OR Weston General Hospital plus our BNSSG system partners at Sirona and NBT. Hours of operation are 0700-21:30

It is essential that you have daily access to an insured car (business insurance) as you will be driving to people's home to provide nursing care. Travel expenses will be paid.

You must also be registered on the permanent NMC register for this post.
