Summary
As a Social Worker, you'll be part of a supportive and friendly team working with other social/health care professionals to support with hospital discharges and moving people to short term care services and onward care providers. You'll complete assessments, develop care plans, and provide people leaving hospital with support and guidance to enable them to maximise their wellbeing. Through your person-centred approach, youll focus on the elements of care, support and treatment that matter most to the person, their families and carers. Using strengths-based practice, you'll focus on what a person can do, whilst finding innovative, alternative community resources to meet their needs. Youll also provide coaching and mentoring support to Senior Case Workers and Case Workers. We offer the opportunity to progress to Senior Social Worker roles through our Excellent Practice Validation programme.
