Job summary

Working as part of our well-established Hospital team, youll have the benefit of engaging with people face-to-face as you work in partnership with Nurses, Occupational Therapists and Consultants in this fast-paced, multi-disciplinary environment. With excellent relationship building skills, youll work closely with other members of the team across other locations, including Community Hospitals and short-term services.

Main duties of the job

As no two cases are the same, youll enjoy a diverse caseload and have the opportunity to really stretch your social work skills. Dealing with a broad spectrum of complex issues, including Mental Capacity Act assessments and safeguarding, your sound understanding of our legal obligations, will support your decision making. Job responsibilities

As a Social Worker, you'll be part of a supportive and friendly team working with other social/health care professionals to support with hospital discharges and moving people to short term care services and onward care providers. You'll complete assessments, develop care plans, and provide people leaving hospital with support and guidance to enable them to maximise their wellbeing. Through your person-centred approach, youll focus on the elements of care, support and treatment that matter most to the person, their families and carers.

Using strengths-based practice, you'll focus on what a person can do, whilst finding innovative, alternative community resources to meet their needs. Youll also provide coaching and mentoring support to Senior Case Workers and Case Workers.

We offer the opportunity to progress to Senior Social Worker roles through our Excellent Practice Validation programme.
