Summary


An exciting opportunity has arisen for physiotherapist to join the Community Response and Rehabilitation Team (CRRT) fixed term for one years.

This is a city-wide MDT service, with the remit of supporting patients in the community to avoid preventable hospital admissions, provide community rehabilitation and to facilitate timely discharge from hospital.

The role includes:
• Supporting CRRT to be an effective single point of access though telephone triage- to identify patient need and priority, providing initial advice, signposting, and redirecting referrals where appropriate using knowledge of wider services,
• Working flexible shift patterns over 7 days to support urgent response, assessment, and intervention, to patients in crisis,
• Working with the MDT to manage both urgent and routine waiting lists,
• Completing comprehensive MDT assessments for a wide variety of patients
• Interdisciplinary working,
• Physiotherapy assessments and treatment plans.
• Supporting the team with ongoing service improvement

The holistic approach of CRRT will require the post holder to work both autonomously and alongside MDT colleagues, social services, hospital and community teams and multiple agencies in providing a high quality of care for our patients. Full support will be given once in post.
