We are at the forefront of transforming stroke rehabilitation services across Cambridgeshire and Peterborough. Our dedicated team is working towards the implementation of the Integrated Community Stroke Service (ICSS) model, aiming to provide seamless, high-quality care to stroke survivors.

This initiative is a collaborative effort, bringing together an established multi-disciplinary team of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists, Specialist Nurses, and Neuro Rehab Assistants. As an Advanced Clinical Practitioner, you will play a pivotal role in providing high-quality, evidence-based, and patient-centred care within the specialty of Stroke Rehabilitation.

This position offers the unique opportunity to lead in the development and enhancement of clinical practices, forge strong connections between various health sectors, and contribute significantly to the advancement of stroke rehabilitation services.

• Deliver high-quality, evidence-based care in Stroke Rehabilitation.
• Lead the development of clinical practices within the Integrated Community System (ICS).
• Act as a key liaison among the Older People's and Community Directorate (OPAC), CPFT, and external partners to innovate and improve stroke rehabilitation pathways and services.
• Support service delivery with flexibility and adaptability.
• Must be a qualified Advanced Clinical Practitioner with relevant experience in stroke rehabilitation.
• Ability to travel independently across work localities; a valid driving license and access to a car for work purposes are essential.
• Demonstrated ability to work effectively in a multidisciplinary team.
• Excellent communication and leadership skills.
