Summary

Assistant Patient Pathway Coordinator
Band 3

An exciting opportunity has arisen within the Community administration team at Croydon University Hospital in our Urogynaecology Department. The post holder will be the point of contact for all administrative issues relating to patients’ pathway of care. The post holder will work as part of a multi-disciplinary Point of Delivery team (POD), and will provide support in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.

The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, customer focussed and knowledgeable point of contact. To provide a consistent approach across the Trust, Standard Operating Procedures (SOPs) will describe how the functions of the role will be carried out, along with the timescales to which are to be adhered.

The successful candidate will be highly motivated and organised with an ability to prioritise and problem solve in a busy working environment. Communication is key for this role and it will be expected that the post holder can demonstrate the ability to communicate with patients and staff across all disciplines.
• Ensure effective communication that transfer GP referrals to the Urogynaecology service
• Work closely with other administrators and clinic staff regarding individual patient queries to ensure targets are met and patients are treated efficiently and equitably
• Monitor and review patient care and/or treatment progress using the Patient Tracking List in line with Trust and local policies
• Receive patient referrals and administer in line with timings and requirements as set out in Trust policies
