Summary
• In respect of outpatient referrals:
• To make referrals to outpatient clinics elsewhere in the Trust as requested by nursing/ medical staff, completing all documentation required for the referral
• To ensure the availability and preparation of patient hospital notes together with the relevant documentation for all patients admitted to the service. To ensure the availability of patient hospital notes when requested for clinical audit/Risk & Governance departments.
• To use the case note tracking system to record the location of case notes, ensuring that the tracking system is maintained and that staff with access to it can locate case notes over a 24 hour
• To liaise with medical secretaries and other hospitals to locate missing case notes and if unsuccessful to escalate to Operational/Clinical
• To complete the electronic discharged
• As instructed by a nurse or doctor, to arrange for any necessary follow-up investigations for
• To collate documentation and file appropriately in hospital case
• To book patient transport when
• To work as part of the wider ward team assisting with general
• To answer the telephone and door in a professional and courteous manner.
