Summary

Specialist Dietitian
Band 6
9 Months Fixed Term Contract- 18.75 hours per week



We have an exciting opportunity for an experienced dietitian to work within the Acute Dietetics team to cover a period of maternity leave.

We are a busy team who enjoy excellent working relationships with SALT and Nutrition Nurse and are a valued member of the ward MDT.

We can offer flexibilty with regards to the hours / what specialist areas you would like to cover and would welcome a conversation to discuss what would work for you.

The role would predominantly involve dietetic assessment and advice to inpatients referred to the service. The majority are for oral nutrtional support or enteral tube feeding, although Parenteral Nutrition can be included.

We can offer the opportunity to cover more specialist areas such as Stroke, Gastro, Surgical and Respiratory depending on your experience.
• To provide a high quality, efficient Dietetic service to adult patients on a variety of wards.
• To provide knowledge and experience to assist less experienced dietitians manage their more complex patients in a safe and high quality way across surgical, medical and elderly care wards.
• To participate in the training of other health professionals as agreed with team lead.
• To cover clinics (critical care, outpatients or community) if required.
• If appropriate, and after training to support dietetic assistant and do 1-1s and annual appraisals with them.

• To be responsible for maintaining accurate and comprehensive records of assessment and treatment in line with British Dietetic Association national standards and local policy.
• To be familiar with the Trust and Directorate organisational structure and lines of communication.
• Use a range of communication skills to establish a rapport, demonstrate empathy towards patients and carers and give effective advice to patients, staff and carers who maybe under stress or who have poor or limited understanding.
• To be able to use and move between appropriate forms of verbal and non-verbal communication with patients, carers and staff.
• To be able to sensitively impart unwelcome news to patients and relatives e.g. permanent dietary change/tube feeding/inability to eat again/long term chronic disease.
• To communicate and negotiate with patients (some of whom may be in denial) to gain consent for treatment and to agree expected treatment outcomes.
• To contribute to the development of a range of evidence based and culturally appropriate educational resources e.g. diet sheets and multi-disciplinary teaching aids.
• To communicate effectively both verbally and in writing with internal and external agencies, e.g. Consultants, GP's, community staff, catering, nurses, Social Services and support services about patient care including professional clinical reports.
• To deal with initial complaints sensitively avoiding escalation where possible, seeking advice and referring to senior staff as required.
• To work with link workers or interpreters as necessary during assessment and treatment to overcome barriers of communication.
• To maintain a calm, professional approach to their duties.
• To exhibit a committed attitude to all aspects of work.
• To undertake an individual comprehensive assessment of patients including those with diverse or complex presentations/multi-pathologies in order to identify their problems and needs and ensure that patient's medical, biochemical, pharmacological, psychological, social and cultural factors are taken into account.
• To undertake accurate measurements to assess nutritional status and body composition including anthropometry, percent weight change and BMI/weight for height etc.
• To interpret treatment regimens, blood test results, fluid balance and stool charts for a range of disease states and monitor changes, adjusting dietary treatment if necessary to prevent under/over-nutrition.
• To be able to use own knowledge of food, and nutritional analysis packages to analyse food intake, recipes and tube feeds.
• To be able to accurately calculate nutritional requirements for critical care patients and those with specialist disease states.
• To advise medical staff/GPs on the prescribing of appropriate products e.g. enteral feeds, vitamins, minerals and other specialist dietary products etc.
• Access advice and support from more senior dietitians as required as part of training and Continuing Professional Development.
• To communicate complex condition related information to clients, carers, other family members, members of the multidisciplinary team, Consultants and other professions and agencies, including recommending changes to drug prescriptions.
• To monitor and review dietetic care plans against outcome measures and adjust care plans to facilitate achievement of goals.
• To be aware of the Mental Capacity Act and Trust documentation in use when assessing capacity.
• To organise and attend patient related meetings e.g. best interest meetings, discharge planning meetings and patient review meetings, ensuring effective communication of the patient's dietetic needs thus ensuring a well co-ordinated and comprehensive care plan, including discharge packages.
• To be able to give talks to groups of patients e.g. cancer support meetings, stroke rehabilitation and to medical / clinical staff.
• To participate fully in own appraisal, development and the PDR process.
