Summary

Clinical community dietitian

Band 6- 30 hours - Permanent position

We are looking for a motivated and enthusiastic Dietitian to join our friendly team of Community Dietitians.

This is an excellent opportunity for a Band 5 Dietitian wanting to progress into a band 6 role or for an existing band 6 wishing to gain experience in new areas.

The post will suit a dietitian who thrives on variety and wishes to experience patient care in arrange of different settings (clinics/ home visits/care homes and intermediate care)

You will maintain skills in a range of clinical areas including nutrition support, gastroenterology, liver and pancreatic conditions, diabetes and other long-term conditions and have a keen interest in nutrition support and tube feeding.

You will carry out an urgent telephone assessment clinic for prioritized patients.

You will enjoy managing patients holistically in their community setting, liaising with acute dietitians, tertiary centers and other health professionals as needed.

You will be an excellent communicator enjoying 1-1 consultations and telephone consultations and enjoy working with other professionals on the inpatient intermediate care unit

Quality improvement initiatives are welcomed in the department. You would be encouraged and supported in considering improved and more effective ways of working.
• The post holder will provide Dietetic treatment for adults based in community settings.
• The service will be provided in a range of locations including clinics, patients own homes, care homes and the intermediate care unit if needed.
• You will be interested in a wide range of conditions and maintaining broad dietetic skills and will enjoy working both with patients and other health professionals attending MDTs where needed.
• You will be interested in supporting patients needing nutritional support e.g. cancer patients, patients with neurological conditions including some tube feeding patients and will carry out telephone assessments on urgent patients to avoid delay.
• You will be involved in appropriate prescribing initiatives, taking the lead and liaising with medicines management if appropriate.
• You will be involved in the training of student dietitians during their placements.

• To provide a quality dietetic service to adult patients ensuring a high standard of care is delivered in line with professional standards
• To carry out detailed nutritional assessments, develop treatment plans and advise for patients with a range of nutritional needs including nutritional support, diabetes, GI conditions and for patients with a range of long-term conditions
• To manage a caseload of both oral and tube fed patients and balance priorities
• To  participate in service development, taking the lead where appropriate
• To contribute to the development and implementation of resources 
• To contribute to policy development, leading where appropriate
•  To work in a multidisciplinary way to change and improve practice when treating patients at risk of malnutrition or with other nutritional problems 
• To be involved in appropriate prescribing initiatives, taking the lead and liaising with medicines management if appropriate
• To participate and lead on audits as needed  
• To take responsibility for ensuring that patients and carers have consented prior to dietetic intervention, in line with professional guidelines.
• To maintain electronic patient records inline with trust policy and professional guidelines
