Summary

Senior Digital Health Assessor
Band 6 - 37.5 hours - Full Time,Permanent

We are looking for Individual Nurses and /or Paramedics with a sound clinical knowledge and experience of an acute setting/background working with Adults you will have excellent assessment skills demonstrating a logical decision making process that ensures patient safety.

This post will require you to utilise your clinical assessment and management skills to enhance the experience of all appropriate patients under our care to access and use the facilities / services available via Digital Consultation & telephone triage assessments.

This post will enable early supported discharge, prevent inappropriate re-admission to hospital, share nursing knowledge and skills and support the technological advances which are being developed at Tameside and Glossop Integrated Care NHS Foundation Trust.

The Senior Clinical Assessor will support and participate in the development of integrated pathways and care packages which enable patients to be safely cared for outside of the acute hospital setting and support palliative / end of life patients through a designated telephone service.

Video assessments

111 calls for adults & children (Training is provided)

Telehealth Service

Virtual Wards

ED streaming (Based in ED)

NWAS Response & crisis response service.

The Digital Health Service is currently being developed to support Tameside and Glossop’s new Model of Care. This post provides the opportunity to lead the way in digital health in Tameside and Glossop be part of a new and emerging team.

This post will require you to utilise your nursing and clinical skill to enhance the experience of all appropriate patients under our care to access and use the facilities/services available via Digital Consultation.

This post will enable Adults & Children to be cared for safely by remotely supporting established triage assessments & care packages at home or in their place of residence.

This post will enable early supported discharge, prevent inappropriate re-admission to hospital, share nursing knowledge and skills and support the technological advances which are being developed at Tameside and Glossop Integrated Care NHS Foundation Trust.

Section 1 - Main Responsibilities

Professional/Clinical
• Provide the delivery of evidence based nursing care by providing expert clinical assessment, care planning, care delivery, care evaluation and clinical advice in accordance withNMC Code of Professional Conduct.
• Ensure all records relating to patient care are accurate, clear, concise, legible and in accordance with theNMC Standards for Records and Record Keeping.
• Maintain a high standard of clinical care for all patients utilising Digital Health in accordance with Trust guidelines and protocols.
• Participate in the provision of early intervention with appropriate assessment, observation and treatment for seriously ill patients within the Digital Hub and within the trust under the supervision and support.
• Support the development of care plans for Intermediate care and complex care Patients, referring to other services as required.
• Work alongside the hospital at night service.
• Participates in practice development programmes and assists with the audit and evaluation of clinical standards.
• Participate in the in-service training, preceptorship and supervision of junior staff, post-basic course nurses and students, with a commitment to self-development.
• Develop good Trust wide working relationships and communicate effectively with nursing colleagues and members of the multi-disciplinary team.
• Promote quality care and non-discriminatory practice as outlined in Trust Policies and Procedures, acknowledging a patient’s personal beliefs and identity

Clinical Expertise
• Demonstrate professional accountability and adhere to the NMC Code of Professional Conduct / other relevant professional association’s code of conduct.
• Take responsibility for the assessment of acutely ill patients who contact the Digital Care Hub service via telephone and video.
• Competently assess and triage acutely ill patients intermediate care patients and complex care patients using the digital care service, including the assessment, planning and evaluation of programmes of care.
• Participate in the multi-professional review of patients who have been referred to the Digital Health Service, including those recently discharged from the hospital.
• Develop competence of professional practice to advice and support patients, using appropriate protocols.
• Develop clinical skills in the assessment of seriously ill patients via the Digital Hub, as well as within the hospital.
• Liaise with the appropriate clinicians/nursing staff to organize the safe and timely admission/transfer of acutely ill patients.
• Supervise and maintain accurate legal records using care planning.
• Offer help and advice to patients’ relatives and friends responding to their individual needs in a caring and supportive manner.
• Collaborate with ward and community staff to provide the appropriate delivery of patient care and clinical and professional advice.
• Adhere to Tameside and Glossop Integrated Care NHS Foundation Trusts policies and procedures relating to Health and Safety.
• Consult and co-operate with medical, nursing and support staff from all areas.

Administrative/Managerial
• Manage the Digital Hub resources effectively.
• Inform the Digital Team Leader regarding accidents, complaints, untoward incidents and other significant events in the Trust and DCH.
• Supervise and ensure the safe use and efficient management of equipment and supplies.
• Ensure the safe handling of drugs, patient’s property and valuables, hospital waste and substances harmful to health, according to the Trust’s policies.
• Provide support to all team members especially in times of stress, learning to recognise this in self and others.

Human Resource Management
• Participate in the training and development of student nurses and other staff as appropriate.
• Participate in clinical supervision for junior staff in accordance with Trust policy.
• Assist the Team Leader to ensure that there is appropriate allocation of staff duties.
• Ensure communication of information of a complex nature is given sensitivity and confidentiality is maintained.
• Ensure that all Tameside and Glossop Policies and Procedures and adhered to at all times and ensure that all Mandatory training is up to date.
