Summary


This is a great opportunity for an experienced AKC Nurse to join our team in a part-time permanent position.

This role will support both the Specialist Nephrology and the Transplant clinic to deliver Advanced Kidney Care.

You will run a nurse-led service and be adept with history taking, recognising and treating symptoms of chronic kidney disease, providing education for renal replacement therapy or managing maximal supportive care.

You will be part of a strong established AKC team that provides care across Northwest London. You will be supported by a Consultant Nephrologist, the lead AKC nurse, the frailty team and the surgical team.

If you enjoy working closely with patients who are approaching renal replacement therapy, helping them decide on their treatment plan, organising their care, monitoring and treating their symptoms, then please apply.

• Provide specialist nursing advice to support and facilitate high quality, individualised care to patients/clients and relatives/carers without supervision in collaboration with the multi-disciplinary team in line with local and national targets
• The post holder will provide specialist nursing care and support and deputise for their line manager in the management of their clinical speciality. This will involve dealing with complex situations and having to take appropriate action in a variety of settings
• To be flexible in the approach to the clinical area and to provide cover for any area specified acting as professional role model providing direct and indirect clinical care, teaching and research.
