We are seeking to appoint to the post of Pathway Coordinator within the Integrated Medicine at Ealing Hospital. The postholder will be working within the Rheumatology / Renal team. This is an exciting opportunity for an enthusiastic and self-motivated individual to join the team.

The post holder will be responsible for providing secretarial and administrative support to the clinical team and the role will focus on supporting a positive patient experience within the service with a key emphasis on patient pathway tracking and validation to achieve RTT standards. The post holder will work closely with the service team and receive support and training as required to fulfil the role.
• To work within the Clinical Support Team to deliver high quality administrative services to designated clinical services.
• Track designated patients throughout their pathway, from point of entry, escalating issues promptly, in order to ensure compliance with waiting time standards
• Support the coordination of national and local audits by effective data collection as required
• To be responsible for providing administrative support to Clinical Teams
• To ensure that every patient pathway is tracked to be diagnosed and treated as quickly as possible, ensuring this is achieved within relevant RTT standards.
• Maintain an up-to-date record of patient tracking and escalations must be kept on the Trust database.
• To support the Service Management team to ensure all administrative process are delivered including data and evidence collection for relevant national audits.
