Summary


As demand rises with increasing health needs and advances in clinical care, we are responding to the challenges whilst continuing to deliver the highest quality of care. We are developing the services that we currently provide to reduce hospital admissions and to facilitate earlier discharge from hospital through the use of technology, ensuring patients have the safest best care that is delivered in their home environment via the Newham Virtual Ward.

The post holder will provide a high standard, efficient, cost-effective Virtual Ward service to patients with a range of acute and chronic conditions hosted by Newham Univsersity Hospital, Barts Health NHS Trust in partnership with acute and community health service provider. Patients with a range of acute and chronic conditions will be remotely monitored. This is to support patient discharges from hospital, prevent avoidable in-patient admissions and reduce pressure on inpatient bed capacity whilst maintaining standards of care that are equivalent to those in hospital. The post holder will act as a support for the Virtual Ward medical team in monitoring patients who are under the care of the Virtual Ward. The post holder will be line managed by, and will work closely with, the lead nurse for respiratory virtual ward to ensure that there is an effective clinical service for the Virtual Ward Service.
