Summary


The Cancer Care Pathway Navigator will form an integral part of the multidisciplinary team in caring for the physical, social and psychological needs of patients and carers diagnosed with breast cancer throughout their treatment pathway.

The post holder will have daily direct contact with patients and their families, providing support, advice, signposting and assistance with the Living with and Beyond Cancer agenda. This will include the arranging and triaging of the holistic needs assessments as part of their core role.

The role will also involve the undertaking of administrative tasks deemed suitable to the role by the Clinical Nurse Specialists (CNS’s).

The job involves multi site working.

To identify the causes of any delays in the cancer patient journey that may affect the patients' prognosis and/or the delivery of cancer waiting time targets. To work with clinical teams and Service Managers in resolving any delay issues within the cancer patient journey.

We require a dynamic and flexible individual with IT, administrative and organisational skills. The post holder will be required to have excellent communication skills and an ability to deal with complex situations with service users and clinicians over the phone and in person. An ability to work autonomously and to respond quickly to requests is required in addition to the ability to use your own initiative.

The candidate will need to commence Holistic Needs Assessment (HNA) with patient for handover to CNS. Therefore the ideal candidate will have experience of working with or supporting patients or clients with complex holistic needs, a good understanding of cancer and advanced communication skills.

• The role will require highlighting any potential delays in the patient pathway and working to mitigate the impact of these problems to the patients. The post holder will be required to work with the senior staff to support and ensure that the patient and carer experience is central to every stage of their journey.
• To support theSurgical Breast CareCNS’s by responding to the noncomplex workload and referring to the CNS’s as appropriate
• Triaging telephone calls and providing basic advice and support.
• Liaising and referring to other appropriate healthcare professionals.
• Arranging and triaging of the electronic Holistic Needs Assessments (eHNA) during the cancer patient pathway. This will include the navigator assisting the patient with completing their eHNA where appropriate. This will ensure that patients are signposted in a timely manner to appropriate services.
• Ensure the appropriate management of patient information and the co-ordination of patient pathways through collaborative working with medical secretaries, MDT coordinators, clinical nurse specialists and other departments as required within the Trust
• To work with the CNS’s to develop and embed the Living With and Beyond Cancer agenda within the Breast Cancer service
• To ensure the appropriate and timely booking of outpatient appointments.
