Summary


A fantastic opportunity has come in Greenwich Community Falls team to work with MDT supporting individuals who are at risk of Falls . The Rehabilitation Assistant (RA) will work, under the direction of allied health professional (AHP) colleagues, as part of the Multi-Disciplinary Team to meet the rehabilitation needs of the service users and their carers, enhancing independence, safety and quality of life. The post holder will work with clients who are at risk of falls in clinics to carry out exercise classes and also visit patients in their own homes to carry out rehabilitation care plans as set by either the Occupational Therapist or Physiotherapist.

Maintain own Client caseload accepting referrals from appropriate sources and act as designated by referral criteria.


• Undertake agreed client assessment using standardised assessment tools, adapting and treating according to the client’s ability/needs.


• To undertake Therapy tasks under the supervision of a qualified Therapist. This may include, teaching use of equipment, teaching personal exercise programmes, supervising walking inside and out and transfer practice.


• To encourage patients to return to social and exercise activities when appropriate and facilitate them to achieve this.


• To assist the qualified practitioner with goal setting and following the treatment plan.


• To instruct in the use of prescribed walking aids if necessary and practice it’s safe use with the patient.

• Deliver intervention with minimal or no immediate supervision from AHP colleagues regularly working as a lone practitioner.


• To assist the physiotherapist/OT or Nurse to run exercise and education sessions.


• Recommend appropriate interventions to promote independence and safety.

Ability to identify/manage and take action to minimise or eradicate risk to patients.


• Prescribe and ensure safe, appropriate, and effective provision, installation and demonstration of stock equipment.


• Develop knowledge of minor adaptation provision.


• To adapt and modify treatment programmes, following discussion with professional staff, where clients often present as non-routine.

• Ensure new team members are adequately introduced to the Falls Team during induction by acting as a mentor and providing peer support.
• Undertake a proactive role in the on-going in-service training programme for rehabilitation assistant.
• Maintain efficient day to day running of the Falls team by carrying out administrative tasks particularly related to record keeping, filing, supplies, errands and message taking, undertaking routine and general light cleaning/ tidying tasks and preparing and cleaning equipment as per infection control guidelines/protocol.
• Keep up to date with current knowledge and advances in Health and Practice.
• Be responsible for own Continuing Professional and Personal development.
• To be an active member of the falls team, participating in staff meetings, in-service training programme and team appraisals and service development.
Clinical• To attend organised mandatory training sessions, including manual handling training, Basic Life Support training and any in-service training arranged by the senior staff.
• To comply with Physiotherapy, Occupational Therapy, and Speech & Language Therapy standards, protocols and guidelines as required by the relevant Professional Code of Conduct.
• To be aware of and comply with Trust and Departmental policies and procedures.
• To be aware of the Health and Safety aspects of your work and abide by policies including the prompt recording and documentation of incidents and accidents.
Research• Undertake a proactive role in the on-going in-service training programme for rehabilitation assistant.
• Provide statistical information as required by Lead Therapist in a timely manner.
Communication• Communicate relevant medical/social information, assessment details, advice and recommendations across agencies following local and professional guidelines for sharing information and consent. This would include Consultants, GP’s Social Services, other health care professionals and voluntary agencies.
• Work in a co-ordinated, multi-disciplinary manner, communicating effectively with other team members and the wider clinical network involved in client care.
• Convey complex and sensitive information relating to clients using skill in situations where there may be barriers to communication i.e., use of interpreters, receptive/cognitive pathologies or excess noise
• Be able to adapt complicated and sensitive information into a simplified form to clients who may be seriously ill or have complex or long-term conditions.
• Promote the Falls Team by participating in informal talks for other agencies and professionals during education and training sessions
• Promote and encourage the ethos of rehabilitation with Care assitants/ carers/ relatives through demonstration, example and support.
