Summary


The ward consists of 30 beds and is led by approachable Ward Manager supported by three Ward Sisters. The ward is a Dementia friendly with one Consultants specialising in different aspects of care for frail and elderly with complex discharges. The care delivered is of a very high standard and the whole team work closely to ensure all patient experience is positive. The ward received a national award of the ELDER FRIENDLY QUALITY MARK in recognition of the support the whole team provides to older people. This mark puts Elizabeth ward in a group of only 40 wards across the country to have achieved the recognition.

Although we have a very strong learning and development function, we appreciate you will need to have acquired some skills and experience from your career you will require in order for your time with us to be a real success.

The post holder will be responsible for supporting the sister/charge nurse on the ward in the management and delivery of patient centred care to patients admitted to the unit. The role incorporates assessing needs, developing, implementing and evaluating programmes of care, supporting and facilitating appropriate rehabilitation opportunities, safe discharges and promoting patient independence.

The role will:

• The role will support the senior sister/charge nurse to ensure that the unit to maintaining a safe and effective service at all times. 1 Communicate and work well within a multidisciplinary team.
• Deliver person centre care to meet the assessed needs of patients and significant others.
• Willingness to expand your elderly and frail knowledge and best practice initiatives.

Main responsibilities
• To use skills and knowledge to contribute to the assessment, implementation and evaluation of patient care.
• To use skills and knowledge to identify subtle changes in condition and to take appropriate clinical actions, including informing the multi-disciplinary team including medical cover of any concerns.
• To ensure care is delivered in a safe and effective manner and as far as is practicable is based on an evidence of good practice.
• To be familiar and efficient with emergency procedures pertaining to community inpatient settings.
• To handover work to the multi-disciplinary team, in an efficient manner, prioritising workload effectively and using appropriate and agreed tools.
• Taking blood samples
• Recording observations
• Medication administration
• Wound care management Risk assessments Safeguarding alerts
• Education and training of junior staff o Involvement in rehabilitation programs
• To maintain up to date knowledge of services which are appropriate to the overall well-being of the patient and to act as liaison between agencies as required.
• To assist in the preparation of high quality written reports and any other written documentation as necessary to support this patient group.
