A wonderful opportunity has opened up for aspiring RMN’s to join ourOlder People Healthy Ageing (OPHA) Home Treatment Team (HTT) serviceat the heart of Kensington and Chelsea, where you will be afforded the time and space to engage in specialised training and development opportunities to help you become a more enhanced community practitioner.

We are looking for experienced, focused individuals, who are highly motivated with excellent communication and managerial skills. Community Services are becoming fast-paced and rapidly changing to support the ‘Out of Hospital Care’ strategies, you will therefore need to demonstrate resilience in the ability to adapt to the changing landscape and opportunities.

You will be working as part of the Community Mental Health team to offer intensive support and treatment to patients in the community in the acute phase of a mental crisis as an alternative to inpatient care. To act as gate keepers to all admissions. To support early discharge from hospitals.

You will be expected totriage referralsand allocate work to junior staff as appropriate in addition to providing a comprehensive mental health assessment for complex patients in the acute phase of a mental health crisis.

You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners and to have close working relationships with the community teams, inpatient services and the HTT

To help you achieve the above our team will also offer you;
• Commitment from the team to develop youby offeringcoachingandin-house developmentsessions such as,Leadership programmes,CBT,DBTand more
• Opportunities to shadowour neighbouring services including CMHTs, acute inpatients, PICU and HBPoS allowing you to see the full circle of treatment for our patients
• Local Reflective Practice forumsrun by one of the senior lead
• Mentoring and coaching sessions with very experienced team leaders supporting you with your development and growth into the role
• Clear objectives and pathway to develop your career further
• HTT service works 5 days a week (37.5 hours) Monday through Sunday, 9 to 5pm. One weekend on and one weekend off shift patterns
• We also feel that our service considers the patient as we have the same consultant working for both teams so that we all have a really good understanding of each individual patient and can work collaboratively together to provide the absolute best outcome for the patient.
• The HTT is a nurse led service with a dedicated medic and is well embedded in the CMHT.
• QI project’s– working closely with UCL in a study around isolation and depression.

• The Older Adult Home Treatment Team has a dedicated workforce including Consultant, speciality Doctor, a Team Manager, Band 7 Clinical Leads, Band 6 and 5 RMNs supported by Band 4 clinical and administrative staff.

The HTT is a multidisciplinary team, comprising CPNs, psychologists, Art therapists, social workers, OTs and doctors. The team is supported by a good admin system and offers an excellent work environment. The team has good links with local statutory and non-statutory services. We are based in the Wealdstone area of Harrow, which has easy links to Central London and good local transport connections. The team works closely with Ellington Ward (Bedded unit) based in Northwick Park Hospital.
• To triage referrals and allocate work to junior staff as appropriate.
• To provide a comprehensive mental health assessment for complex patients in the acute phase of a mental health crisis.
• To assess current risks and identify how the risk and the patient's mental health needs would best be met.
• To formulate, implement and review individualised Recovery Care Plans with patients and their carers that aims to address their psychological, physical & social needs.
• To build and maintain therapeutic relationships with patients and carers and involve them in drawing up care plans.
• To work with patient care co-ordinator/ lead professional as appropriate to ensure continuity of care.
• To provide information and advice to carers to support them through the crisis period.
• To administer, supervise, and monitor prescribed medication advising patients and carers on storage, administration and possible side effects.
