Summary


An exciting opportunity has arisen for a Full Time Band 3 Support, Time, and Recovery Worker to work within the Warrington Later life and Memory Service (LLAMS)

This is a fixed term 6 month opportunity.

The hours for this post are 37.5 hours per week, part time working hours will be considered.

The operational hours are Monday to Friday 9am – 5pm the base for this post is Hollins Park site.

The role of this post is to assist in outpatient clinics, completing cognitive testing, and physical observation, and supporting CMHT with home visits, and medication side effect monitoring and compliance.

1. To assist in the assessment, planning, implementation and evaluation of programmes of care for service users, including health care assessments under the direction and supervision of qualified members of
2. To assist in the facilitation of medical review clinics, under the direction of qualified members of
3. Contribute towards reports required by managers or external agencies, for example child care conferences or court
4. Working as part of a multi-disciplinary team in a non-judgemental
5. Assisting in the facilitation of group work with service users, under the direction and supervision of qualified members of
6. Participate in and perform clinical investigations under supervision of qualified members of staff.
7. Establish and maintain communication with people on routine and operational
8. To respond in a courteous and professional manner with clients, visitors, other agencies and their
9. Contribute towards the maintenance of accurate client records according to Trust
10. To liaise with staff from other agencies, taking into consideration confidentiality
11. To contribute to effective running of the service by providing and receiving information to/from other departments or services. Ensure this information is provided in the most effective way i.e. telephone, fax or in
12. To assist service users with self-administration of oral medication and ensure any concerns/issues about medication are reported back to a qualified
13. To use a variety of verbal and non-verbal skills to overcome communication barriers presented by service users due to their mental health
14. To develop and maintain links with other agencies and services, both statutory and voluntary.
15. To use skills and techniques to de-escalate situations where service users become physically or verbally
16. To contribute to the development of risk management plans for service users by reporting to the qualified practitioner any issues or concerns regarding service users behaviour/action towards themselves or
17. To adhere to the Trust’s rights based approach taking into account service user
18. To take responsibility for ensuring your own personal safety and that of the service user when using
19. To take reasonable care for the health and safety of yourself and any others that may be affected by your own acts and omissions at
20. To ensure that you are familiar with the terms and conditions or your
21. To participate in service development activities, as
22. To participate in the services’ performance management supervision framework and work towards meeting identified goals and objectives as agreed in personal development
23. To act at all times, in accordance with Trust policies and

25. Under the guidance and supervision of a qualified practitioner, to make accurate and appropriate entries into health
