Summary


The Care Navigator service in Manchester is part of the Integrated Neighbourhood Teams of which there are 12 across Manchester , 4 in each Locality of Manchester. The service offers a community facing service with focus on delivering a seamless interface between primary care, hospital and community health , social care and other statutory services, ensuring the people of Manchester have access to the right services at the right time in the right location.

The post holder will proactively work together with services across boundaries to ensure people are kept in the community through reducing the need for formal health and social care provision. Activities will include admission avoidance/prevention alongside community teams, admission deflection at the front door of hospitals and facilitated discharge from the hospital wards.



The Senior Care Navigator has responsibility for ensuring appropriate caseload management across the Locality, and responsibility for planning to meet capacity and demand across the service.

The Senior Care Navigator will work closely with the Citywide Care Navigation Lead and Integrated Neighbourhood Leads to develop and implement new pathways to support the co-ordinated care pathway.
