Summary


The post holder will be responsible for the co-ordination of the patient pathway through the MFT Stop Smoking service The successful individual will liaise closely with the clinical teams. He/she will work independently to collect and provide all information required by the clinical team. They will be responsible for ensuring all communication between MFT and external trusts is efficient and timely and will be encouraged to devise innovative ways to manage such information. The post holder will also be responsible for the organisation and co-ordination of populating the stop smoking service booking/triage.



He/she will be an integral part of the Rapid Cure team, supporting the team in meeting all requirements set locally, nationally.

He/she will monitor and track patients within the agreed specialty(s), liaising with all staff levels to assist the patient’s journey, the clinical team and the trust in meeting the standard key indicators which currently form the national star ratings.

He/she will work independently, planning, and organising own time, design and implement systems to improve and develop the MFT Rapid CURE team, including the Lung Screening Programme.
