Summary


Come and work for NELFT’s inpatient (KMAH) and crisis resolution and enhanced support teams (CrEST) as our dedicated lead for patient flow through our pathway.

KMAH is a 14 bedded general adolescent unit including 3 eating disorder beds. We also have an onsite health based place of safety (HBPOS) and in the process of opening a short stay service too. We have a wonderful ward staff group and a highly skilled full MDT that work fluidly through our inpatient service and into CrEST.

CrEST includes our crisis assessment and triage team (C.A.T) and he intensive home treatment team (IHT). C.A.T operates a 24 hour crisis response and paediatric liaison service across Kent and Medway providing support and advice to patients and carers, as well as receiving referrals for advice and assessment from key partners in care, including the police, Ambulance Service, Single Point of Access and acute trusts. IHT work intensively with children and young people experiencing acute mental health difficulties who would otherwise require hospital admission. We support young people in the community for up to 12 weeks to prevent admission to a tier 4 service and support early discharge from inpatient services.

As the lead for patient flow throughout the whole pathway you will play a pivotal role in ensuring smooth transitions, timely admissions and discharges within the service and building key relationships with system partners and the provider collaborative to develop safe and effective care and treatment plans for our patients.

You will lead on monitoring and managing referrals into KMAH ensuring timely responses to promote patient safety and appropriate access and waiting times to admission. This will also include completing access assessment and pre-admission assessments and advocating for least restrictive interventions for our patients.

Within the role you will work closely with the unit clinical team especially the consultant, named professional and social worker to proportionately share safeguarding risk and promote safe discharge alongside the external network.

There will be an expectation of the lead to hold and chair professionals’ meetings, escalation calls for patients who are declared as delayed discharges (including patients in the HBPOS and short stay beds when open), attending CETRs (care education and treatment review) and attending the weekly clinical activity panel meeting with the providor collaborative.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.



There are a number of pathway leads within KMAH and CREST and there will be an expectation to work in collaboration and alongside each other to ensure the pathway is covered from an operational and clinical perspective. This would include offering operational and clinical oversight and expertise to all services including KMAH, HBPOS, IHT and the C.A.T team.

You will be closely supported and supervised by the modern matron within KMAH.

This job would be suitable for a nurse, social worker or OT.
