We have an opportunity for a qualified and skilled Team Leader to join our Community Nursing Team.

The community nursing service provides case load management for housebound patients to support their nursing needs and enabling them to self-care. Nurses within the team are supported to develop advanced skills to manage the complexity of patients seen.

Community nurses are also supported through the Specialist Practitioner course, recognising community nursing as a specialism within the NMC.

As a Band 6 Team Leader you will work closely with the people who use our services, families, friends and carers and play an important contribution in how people experience our services.

You will also be acting as lead and senior practitioner to support junior staff carry out assessment/reassessments of service users and/or pathways with complex needs across the relevant field.

Your patients could be any age, but they'll often be elderly, while others may have been recently discharged from hospital, be terminally ill or have physical disabilities.
