Summary


REABLEMENT SUPPORT WORKER

Base: Laurel Ward

Based on the older adult inpatient ward in Scunthorpe, you will be joining a friendly and supportive multidisciplinary team and have an opportunity to contribute to service quality and development. Laurel Ward is a 13 bed inpatient ward providing care and treatment to predominantly older adults with a functional or organic mental health diagnosis.

You should have experience of working with older adults experiencing mental health problems, including experience of working with people with dementia.

You should be committed to providing person centred care and positive risk management, putting our patients at the heart of everything you do and ensuring their independence and activity of daily living skills are promoted every step of the way. This role will include some weekends and evenings as well as during the week, this will be on a 4 week roster basis.

You will work as a member of our multi-disciplinary team alongside a variety of qualified and support staff as well as developing links with external agencies and community resources to provide high standards of care for services users and support for their families.

You should be a confident and effective communicator with patients and carers, staff of all professional groups as well as with external partner agencies.

You should be passionate about the contribution of activities in mental health services for older adults and have a specialist interest in dementia care.

To maintain independence and daily living skills of inpatients by providing high quality therapeutic group and individual activities under the direction of the Occupational Therapist and Nursing team.

To provide an activity program for the ward that will meet the needs of patients who have cognitive and functional illnesses, this is to be done in liaison with our Occupational Therapist and Physiotherapist.

To contribute to specified individual assessment activities as directed by the Nurse in Charge including personal care and domestic assessments.

To provide information to the ward team to enable ongoing assessment and contribute to safe effective discharge plans. Activities may be undertaken in community settings as directed.
