We are currently looking for a motivated and experienced Registered Learning Disability Nurse to work as Lead Nurse supporting the Personalised Healthcare service who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for individuals being assessed for Continuing Healthcare (CHC) funding or who are in receipt of CHC funding.

This is a key position within a changing service and provides a good opportunity for the successful candidate to gain experience and learn new skills.

The post holder should hold a full UK driving licence and have access to a vehicle to be able to fulfil the requirements of this post.

The post holder will play a key role in supporting the Clinical Manager in managing and developing the Continuing Healthcare team, working in conjunction with external stakeholders, to implement a local vision of Continuing Healthcare services, ensuring the delivery of key quality outcomes and strategic objectives and Continuing Healthcare services across the region.

This is a team leader post in the Personalised Healthcare Service and will lead professionals in coordinating Continuing Healthcare as well as developing policy alongside key stakeholders. They will support the Clinical Manager in ensuring work is planned and managed effectively and will take a lead in successful delivery of services. The post holder may be required to undertake CHC assessments and reviews.

Our hours of work are Monday -Friday, 9am -5pm with the opportunity for agile working. MLCSU is committed to the personal and professional development of their staff and offers a wide selection of learning and development opportunities.

Currently the post is agile working with a requirement to attend the office base as and when requested. All assessments will be face to face, a vehicle for the purpose of business miles is essential for this role.

• Leading and managing a team of Continuing Healthcare Assessors ensuring a high-quality service is delivered, supporting and developing staff to achieve continuous improvement.
• Maintaining constructive relationships with a broad range of internal and external stakeholders including Health and Social care Services.
• Contributing to performance improvement, taking a lead for identified areas where agreed, and overall day to day management of the Continuing Healthcare team performance.
• Supporting staff to complete all mandatory, PHB and other training so that staff can fulfil their roles effectively.
• Managing recruitment process for Practitioners and Senior Practitioners.
• Contributing proactively to the management of resources by ensuring that all Continuing Healthcare decision making is properly recorded and maintained.
• Implement safe decision-making processes, enforced by a robust policy, that is based on national best practice, which ultimately supports the safe authorisation of PHBs.
• Analysing, interpreting and communicating complex national directives into processes, policies and presentations for application.
• Planning and deploying resources to manage individual and team performance to meet the demands of the service.
• Overseeing the management and resolution of complaints, appeals and disputes, ensuring operational staff complete the responses as necessary and to a high standard.
• Raising issues of concern through the Senior Management Team where necessary.
• Overseeing the delegation of workload within the team.
