Summary
Seeking a Field Care Coordinator who is dedicated, disciplined and empathetic towards clients.

Key responsibilities:
• Facilitating communication: liaising between various healthcare providers to ensure that each client’s care plan is comprehensive and cohesive.
• Developing care plans: creating detailed care plans based on client needs, involving input from patients, their families, and healthcare providers.
• Work closely with both the Care Manager and Care coordinators as well as other office staff.
• Participate in an on-call rota.
Essential requirements:
• A valid driver's license and/or access to a vehicle
• Strong experience in the care industry as a senior carer/field care coordinator
• Level 2 NVQ in Health & Social Care
