Summary


We are seeking to recruit Transfer of Care Hub System Discharge Leads who will lead a system-level Transfer of Care Hub (ToCH) for the Brighton & Hove ‘place’. Transfer of Care Hubs will link all relevant services across sectors to aid discharge and recovery as well as admission avoidance. The Hub will bring together all relevant services (acute, community, primary, and social care, and housing and voluntary sectors) to coordinate care and support for people who need it during and following discharge.

The successful post holders will lead the continuous development and improvement of the Transfer of Care Hub in line with evolving National Hospital Discharge guidance and good practice. They will support a collaborative approach to understanding requirements and effective service delivery to support discharge and admission avoidance based on the health and social care needs of the population and the social care needs of people across Brighton & Hove.

This post is a ‘system’ post, hosted by Sussex Community NHS Foundation Trust, but with equal responsibility and accountability to all system partner provider organisations involved with discharge. Working with all system partners and beyond, the post holder will take a lead role in the day-to-day development and leadership of a multidisciplinary discharge service, ensuring local and national targets are achieved. The role will provide visible leadership for all areas of responsibility and provide direction to all staff working within the ToCH from different organisations.

You will already be a senior leader with experience of working with multiple stakeholders across numerous different provider organisations. This is an ideal role for someone wishing to develop in their career and use their leadership skills, whilst being supported by experienced senior leaders from across the health and social care system.

For this role, the post holders will need a health or social care related degree at Masters level or able to demonstrate substantial knowledge and experience within the relevant field with a willingness to work towards qualification. They should also hold relevant accredited professional qualification (e.g. Social Work, Therapy, Nursing) or working towards this.

The base in Brighton and Hove will be Royal Sussex County Hospital. Please be aware that the post holders will be expected to travel to other sites across their area.
