Summary


The Community Therapy Hub and 'in-reach' Rehab Beds Team are pleased to offer an exciting opportunity for a staticband 5 Occupational Therapist to become a part of our therapy team.

We have two work streams within the therapy Hub: one, supporting the in-reach rehab beds where we work closely with physiotherapists and therapy assistants to carry out rehab and discharge planning.

The other; carrying out visitswithin patients homes, providing assessment and interventionwithin the Therapy Hub.

To practice as an Occupational Therapist as part of either:

The in-reach therapy beds team or thecommunity therapy hub. Managing the therapy case load within the beds and delegates work to other team members as appropriate.

To develop and maintain a high standard of clinical practice as a member of the multidisciplinary team, in line with the code of professional conduct of the College of Occupational Therapists.

To assist the multi-disciplinary team in preparing intervention plans, implementing rehabilitation and facilitating discharges.

To work jointly with other professionals to ensure a patient-centred approach.

Review and reflect on your practice through regular participation in professional supervision with your nominated occupational therapy supervisor. To provide clinical supervision to other members of the team, as agreed with the Therapy Lead.

To ensure the maintenance of assessment and treatment equipment in a satisfactory condition, and ensure careful use and return of same, cleaned, where appropriate.
