Summary

To support a caseload of individuals accessing the Complex Emotional Needs Hub, some of whom will have multiple health conditions barriers. At times being exposed to distressing and emotional circumstances, experiencing unpleasant working conditions or having to support individuals who may have barriers to understanding. Sharing and receiving highly complex, sensitive or contentious information. To liaise closely with the Complex Emotional Needs Hub in managing risk or relapse in order to safeguard individuals and relevant others when potential risk of relapse or harm may be evident. To actively participate in multidisciplinary team meetings, reviews, professionals meetings, liaising with other professionals to ensure a seamless service is provided. To liaise and communicate regularly with the referrer and other relevant healthcare staff to ensure that all parties are kept informed and co-operate in helping the individual achieve their goals. To carry out holistic assessments, identify recovery goals and develop action plans in agreement with the individual. Providing personalised support to individuals, their families, and carers to take control of their wellbeing, live independently and improve their health outcomes. Developing trusting relationships by giving individuals time and focusing on what matters to them. To maintain detailed and accurate case notes, action plans and paperwork and upload these on to the clinical management information system within the allocated timeframe. To provide information and support to individuals about the pathways of opportunities available to them to achieve their recovery goals and build a meaningful and hopeful future. To support individuals to gain accurate information and to engage with internal and external services which will support in them achieving their goals. To liaise with and arrange meetings with health professionals, partner agencies (voluntary and statutory sectors) and/or employers, educations providers concerning the individuals needs, including partner organisations that work with diverse groups to develop joint working that will enhance service delivery and increase opportunities for individuals. To contribute to the delivery of training and workshops to individuals on a range of topics to support them in their recovery. To contribute to any informal training with colleagues and external organisations to help raise awareness and improve their knowledge about this service. To undertake referral generation and networking to promote the service. To collect and collate statistics that supports the ongoing monitoring of service delivery/ quality. To provide information for service reports as required and participate in regular audits and evaluations. To work as part of a team to further develop quality standards to continue to improve service delivery. To be responsible for own professional development and participate in continuing educational and in service training. To demonstrate an awareness of current developments within mental health and recovery. To participate in clinical, caseload and management supervision To offer support to the Administrator and to new team members To work flexible hours as required to meet the needs of the individuals and service delivery Along with your main duties, you will also be expected to carry out any other duties that are reasonably asked of you.
