Part time Children Looked After Nurse - Band 6

22.5 hours per week, permanent role

Applications are invited for an enthusiastic and experienced Children’s Nurse or Public Health Practitioner to join our Children Looked After team in Coventry & Warwickshire Partnership Trust.

Directly support Children Looked After (CLA) and those leaving care to reach their full potential by accessing the best possible health outcomes.

Ensure Children Looked After receive timely, child & young person focussed assessments and interventions in line with Statutory Guidance, as part of a caseload covering 0-18.

Support the Named and Specialist CLA Nurses in ensuring Children Looked After receive appropriate assessments and health provision when they are placed Out Of City (OOC) and are placed In To City (ITC)

Support the Named and Specialist CLA professionals, in providing a needs-led, outcomes focussed, accessible health offer to CLA CYP.

To provide training and support; including the provision of specialist advice to Coventry community professionals in relation to improving the health and well-being outcomes of Children Looked After (CLA).

Undertake audit for internally & externally provided health assessments as required, as part of the delegated roles within the CLA team.

The successful candidate will be supported in the role by a clinical education team and clinical nurse specialists as well as the operational management team. You will benefit from ongoing professional development as well as support via clinical and managerial supervision.

The role of the Children Looked After Nurse is to work with other Health and Social Care services to support specialist health care delivery within a multi-professional Children Looked After team. We are looking for an experienced practitioner who will act on behalf of, and work with, Children Looked After & young people to promote their best health outcomes.

Clinically, you will be expected to manage a delegated case load, conducting statutory Review Health Assessments (RHA’s) and generating Looked After Care Leaver’s Health Summaries for Children Looked After living inside and outside Coventry. You will also act as a point of contact for professional liaison and consultation.

You will be expected to work within the local specification and national guidance to respond to Children Looked After and young people, their Foster Carers and others supporting them. You may be required to travel across the country to engage with Coventry’s Children Looked After who are placed out of city, occasionally.
