Summary


An exciting job opportunity has arisen for a named nurse to lead the Children In Care Service in Lincolnshire Community Health Services.

The Children In Care Service provides a quality specialist health service to meet the health needs of vulnerable children and young people aged 0-19 within Lincolnshire.

The post involves managing a small team of nursing and administration staff to coordinate the initial and review health assessments for children placed in care within Lincolnshire.

The post also involves working with partners across the system in Lincolnshire to address health inequalities for children in care to help improve outcomes for these children.



Please note, we will be using Personal Profile Analysis as part of the interview process and will require you to complete a PPA assessment prior to interview.

The post holder will be responsible for the management of a small team of nursing and administration staff with responsibility in implementing quality health services to meet the health needs of children in care within Lincolnshire, working with the Designated Nurse for Safeguarding Children, Designated Doctor for Children In Care and Designated and Named Doctors for Safeguarding Children and the Safeguarding Children Team.

To role will also involve supporting the LCHS designated nurse for safeguarding children in executing the duties and responsibilities of the named nurse role for children in care within LCHS in accordance with the Children Act 2004/Working Together to Safeguard Children, and ‘Statutory Guidance on Promoting the health and wellbeing of looked after children’ and other relevant documents.

The role also involves working in partnership with health, education, police, secure unit staff and social care agencies, and support, develop and implement the training of foster carers, residential staff, social workers, health visitors and school nurses to improve health outcomes for Children In Care.

The Post Holder will be visible throughout the community and will provide regular training and structured supervision including specialist advice and support in response to staff needs.

The post also involves acting as an expert resource / advisor for LCHS to promote a high standard of clinical practice to achieve better outcomes for children and young people in care.

To manage a team of staff in implementing quality health services to meet the health needs of children in care within Lincolnshire.

To support the Designated Nurse for Safeguarding Children, and to contribute to the strategic and operational planning within LCHS in relation to promoting a high standard of clinical practice to achieve better outcomes for children and young people in care.

To contribute to the development of policy and procedures that reflect the statutory requirements and recommendations of all local and national reviews / inquiries and with Lincolnshire Safeguarding Children Board guidance. Participate in ensuring that procedures are distributed, understood and implemented by alerting Managers and Professionals of any changes in light of new National and Local developments

To work with residential social workers, foster carers and service leads within CYPS to promote healthy care standards.

To ensure that the performance Indicators in relation to children and young people in care are met.

To provide expert advice on clinical practice issues to ensure safe best practice.

To collate and monitor performance data relating to children and young people in care and contribute to reports for senior management including level and type of vulnerability factors and analysis of need and outcomes within LCHS.

To contribute to reports for the LSCB, Lincolnshire ICB and Care Quality Commission

To develop and maintain a robust system of administration for children in care, case monitoring; including ensuring that audit cycles for standards of children in care health assessments, health outcomes of children in care, Health Care plans are undertaken within agreed audit timescales.

To contribute to the reduction of health inequalities in Lincolnshire of children in care and enable children and young people to achieve their optimum potential and improve the health of children and young people at risk of social / educational exclusion.

Support the coordination of health assessments and progression of health care plans for children in care who are placed outside of area in collaboration with Local Authority CYPS.

Undertake risk assessments of organisational ability and advice upon appropriate management and record on the LCHS ‘Risk Register’ to ensure safe working practices.

To promote a highly visible role and raise the profile of children and young people in care within the Trust.

To promote good practice and effective communication within LCHS and between LCHS and other agencies on all matters relating children in care and in conjunction with relevant LCHS colleagues ensure issues relating to information sharing and confidentiality are addressed.

Consult with children in care and ensure their views and experiences shape the future development of health services.

To maintain effective communication pathways with all other agencies, statutory and voluntary and outside community health areas, particularly with regard to the transfer of information in and out of LCHS to reduce the vulnerability of children and young people following placement changes.

To collaborate and work closely with the Designated Nurse, Designated Doctors and Named Professionals across Lincolnshire Trusts to ensure there is a consistent and co-ordinated approach to promote the welfare of children and young people in care, practice across the health community.

To take an active role in protecting children and young people in care where there are safeguarding/child protection concerns.

Monitor the delivery of personal health care plans for children in care, ensure they become an integral part of the child/ young person’s care plan and personal education plan.

To provide support and specialist advice on complex situations with staff to develop competence and confidence in the management of children in care and safeguarding children in vulnerable situations.

To maintain and develop own clinical and professional knowledge, skills and expertise and demonstrate a commitment to life-long learning through appropriate academic study which supports Standards of Professional Practice, PREP, Code of Conduct and NMC registration.

Maintain own individual training record which incorporates up-to-date evidence of clinical competence and prepare and provide evidence for own development review in relation to KSF post outline.

To undertake regular Safeguarding / Vulnerable Children and Young People supervision and to be professionally supported, advised and led by the Designated Nurse for safeguarding children.

Provide Safeguarding/ Children in care supervision, professional support, guidance and advice to individual practitioners within the team.
