We are seeking an enthusiastic Band 6 Occupational Therapist to join the Integrated Stroke Early Supported Discharge and Community Neuro Rehabilitation Service.
The Integrated service provided both stroke rehabilitation and community neuro rehabilitation supporting service users with a variety of complex needs.
This is an exciting opportunity to join an established multi-disciplinary team comprising of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists, Specialist Nurses, and Neuro Rehab assistants.
This role requires the successful candidate to manage a clinical caseload across the county of Cambridgeshire. Their base will be Princess of Wales Hospital, Ely.
The role involves visiting patients in their homes, wards, or community gyms. There may be times when there is a requirement to provide clinical cover elsewhere within Cambridgeshire. The role requires agile working often using virtual consultation to manage a caseload. We will provide a smart phone and laptop to support this role.
You will provide comprehensive assessments and deliver evidence-based specialist neuro occupational therapy programmes. You will work as part of a multidisciplinary team in supporting patients with physical, cognitive, and psychosocial needs to achieve their potential.

• To assess, plan and develop specialist OT programmes for patients with complex neurological problems and a wide variety of complex needs with support from advanced practitioner when required.
• To work as an autonomous practitioner within the integrated multi-professional Integrated Stroke ESD and Community Neuro Rehabilitation Team
• To be responsible for own caseload and undertake assessment of patients with complex neurological problems and multiple pathologies using specialist clinical reasoning skills.
• To identify patient needs, agree goals and provide appropriate therapy interventions relating to their physical, functional and cognitive needs. Provide adaptive equipment, rehabilitation programmes, and work with other OT colleagues to make recommendations for minor and major adaptations in line with current legislation.
• To undertake holistic assessment of patients with neurological problems (including those with complex presentations and cognitive problems) making use of specialist clinical reasoning skills.
• To use evidence-based practice, to ensure clinical care is effective and appropriate.
• To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.
• To work effectively and liaise appropriately with members of the MDT and other agencies by attending MDT meetings, case reviews.
