Summary


Are you an acute children’s nurse who’s looking to branch out into community nursing? Or a Children’s Community nurse looking for more acute focussed nursing care at home?

The Royal London Children’s Hospital is looking to recruit or second a band 6 children’s nurse to cover maternity leave to support delivery of the innovative and exciting hospital @ home service commissioned by Tower Hamlets CCG. The team visit acutely unwell children in the community to offer treatment preventing them from attending hospital and/or staying in longer than needed. The H@H team forms part of the Pan London H@H forum with its additional support and links that helps to provide a cohesive and high quality service to the children of Tower Hamlets.

The successful candidate will have excellent team working and decision making skills and put their patients first. We are looking for experienced and highly skilled children’s nurses who can work autonomously to care for acutely unwell children at home. • To facilitate efficient and effective patient flow from attendance to hospital to referral home, helping to ensure that we meet the 4 hour wait target in the emergency department.
• To perform highly skilled acute nursing care and assessment for children at home
• To work closely with the ward managers, paediatricians, matrons and flow team on a daily basis to expedite referrals home
• To act as an excellent role model and leader to all staff
• To participate in the development of new pathways of care
• To support the evaluation of the service regularly and adapt as necessary
