Summary


An exciting opportunity has arisen for an enthusiastic, forward thinking, highly motivated, dynamic, Occupational Therapist to join our established innovative, Intermediate Care Team in Bury.

You will be joining a well-established and supportive team, where person-centred care is at the forefront of everything that we do. As a Band 7 Therapy Lead working within the Intermediate Tier, you will play a vital role in our MDT; managing a complex caseload of adult patients, as well as providing clinical leadership for the service.

To work autonomously within the Trust policy and professional practice and maintain accurate and comprehensive up to date records.To perform specialist assessment and treatment of patients within the specialty who will have complex and/or acute or chronic conditions.To determine a clinical diagnosis, develop, deliver, implement, evaluate and modify individualised treatment programmes, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required.To take a major role in assisting the Service Manager in clinical governance action plans and in the planning, coordinating, delivery and evaluation of the Service within the speciality.To comply with statutory requirements for Physiotherapy and Occupational Therapy, within HCPC standards and codes of professional conduct.To educate and train qualified and non-qualified healthcare staff up to and including medical staff.To deputise for the Service Lead when required. Supervision through peer support and is largely self driven.To work as a lone practitioner in a community setting .
