Degree Qualified Social Worker with 3- 5 years Experience
Neath Port Talbot Adult Social Services is seeking to recruit a Hospital Social Worker to work within the Hospital Assessment Team based at Swansea Bay University Hospital.
The role has become available to join a committed and motivated Hospital Assessment Team who work hard to deliver excellent standards in a positive working environment.
The Hospital Team works in partnership with individuals/ multi professionals to identify what matters to them.
This includes undertaking Mental Capacity Assessments, Continuing Health Care Assessments, leading Best Interest Meetings to ensure people have a safe and timely discharge from hospital.
We work alongside a range of professionals within the teams, including colleagues from the health board, therapies, external providers and families.
It is an exciting time to join adult social care with friendly, experienced and supportive colleagues who are dedicated to providing a service that puts citizens at centre of our work.
