Summary


To undertake practical support for discharge planning within the neurosciences (neurology/neurosurgery) to reduce length of stay, delayed transfers of care and increase the number of timely discharges.
To ensure the discharge planning process is responsive to the needs of patients and their carers by means of providing supportive discharge care.
To be visible at ward level to ensure that discharge planning is proactive and timely and that patients are prepared and supported for their expected discharge day and time.

The Neurosciences directorate at St George’s University Hospital serves a population of 3.5 million and the Trust has an established international reputation as a leading hospital for specialist care including Neurosurgery, Neurology, Stroke and Trauma.

We are looking for a highly motivated, well organised, enthusiastic individual to work flexibly as a full time 37.5 hours/ week Discharge Support Worker within the Neurosciences Directorate.

You will be responsible for improving the patient’s experience and supporting effective, timely and safe discharges by means of providing practical support to the neurosciences discharge team and ward staff in accordance with departmental and Trust policies and procedures.

You will be expected to have excellent communication and organisational skills and the ability to work autonomously as part of a multi-disciplinary team.

Requirements
• Minimum experience of 24 months healthcare/rehabilitation/ therapy assistant or adult social care experience
• Excellent communication skills
• Adaptable and flexible approach
