Summary


An exciting opportunity has arisen to join the Community Matron Team/Hub based at Westhaven Community Hospital. The post holder will work together with primary care, secondary care, community services and social services to provide enhanced and pro-active care as part of an integrated team.

This challenging role offers the post holder the opportunity to provide high quality patient focused care to frail, complex, older people in Weymouth and Portland. The successful applicant will be expected to assess and provide care to patients in their own home and engage and act within a range of services delivered in the Hub, including virtual wards, rapid access services and care closer to home. You will be expected to collaboratively work as part of a multidisciplinary model with General Practitioners, ACPs, Allied Health Professionals, Pharmacists, Health and Social Care Coordinators, Adult Social Care, support self-care, develop appropriate plans, minimise health deterioration via early intervention and work collaboratively with the wider multidisciplinary team to improve outcomes.



Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.


The Practitioner will be required to provide proactive and reactive assessments to prevent unnecessary hospital admissions by supporting individuals to remain in their own homes. They will undertake holistic assessments at the point of need and assist patients and their carers to proactively anticipate their future health and care need. .

Successful applicants must have a broad experience in nursing. They will be motivated, enthusiastic and have proven experience of working as an autonomous practitioner
