Summary
To support in the management of the repeat prescribing process, helping to manage repeat prescription requests, resolving queries where possible and producing prescriptions for clinical review and signature. To ensure patients have appropriate monitoring tests in place when required. • To undertake medicines reconciliation of hospital letters and discharge summaries in line with local protocols and within competency levels. Undertake medication use reviews Day to day handling of prescription queries within area of own competence Provide technical and administration support to clinical workforce and practice administration team (including prescription clerks) • To promote and implement electronic repeat dispensing in appropriate patients; to promote online ordering of prescriptions. • To work alongside the PCN clinical pharmacists to deliver certain aspects of the PCN Direct Enhanced Service (DES), e.g. structured medication reviews (SMRs). • To undertake medication audits in priority areas, such as QOF, NICE guidance, and other agreed target areas, feeding your findings back to the network teams and implementing actions as appropriate. • Provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate. • To take a proactive approach in the safe handling of prescriptions. Undertake training and development, to ensure the required knowledge and skills for practice-based work. Support self-care agenda through patient education on the importance of compliance and adherence to medication regimen and further provide holistic advice to manage patient health and well-being Develop and maintain key working relationships with: • PCN Pharmacists • Practice GPs, Practice Nurses and Healthcare Assistants • Patients and Carers where appropriate • Practice Management, Reception and other Administrative staff • Community Nurses and other allied health professionals • Community Pharmacists and support staff
