Summary


We are looking for enthusiastic, highly motivated rehabilitation assistant to join our Community Rehabilitation Team based at Alexandra Park Health Centre. The vacancy is for a 18.75 hour post in the Integrated Community Rehabilitation Service.

You will be required to work on your own as well as working as part of a an MDT team, to provide rehabilitation interventions for people living with variety of health conditions. This will include delivering communication, cognitive and exercise rehabilitation plans under the supervision of qualified staff.

You will be expected to follow set processes and prioritise your own workload in order to fulfil the requirements of the post. Excellent interpersonal skills and communication skills are essential plus an ability to deal with highly sensitive and confidential information.

Basic IT skills are required and the post holder must have a pro-active, flexible and adaptable approach to their work in order to meet deadlines and have the ability and resilience to respond to changing demands on the service.

Work with the multi-disciplinary team to provide rehabilitation for people living with wide variety of health conditions

Carry an independent caseload and deliver therapy programmes delegated by qualified multi-disciplinary team staff

Organise and provide therapy in exercise classes with other rehabilitation assistant colleagues

Assist with the collection of outcome measures data / audits
