Summary


As part of our commitment to excellence, we are seeking a motivated and experienced Discharge Coordinator to join our dynamic team. Working collaboratively with ward-based multi-disciplinary teams and the discharge hub, you will play a crucial role in facilitating the safe and efficient discharge of patients.

The post holder will also ensure the provision of the specialist nursing advice and the development and implementation of agreed policies for the management of the discharge process. This will include ensuring all long length of stay over 14 days have clear discharge plans and all patients have a Reason to Reside check. This also incorporates the LOS report and weekly meeting and any other relevant P1 meetings
